There will be a once in a life time payment of Rs 10,000/- (Rupees Ten thousand only) for the Indian applicant and $ 300.00 for the Foreign applicant. Payments can be made to DENTAL BUSINESS SOLUTIONS Account no 30450928544 ,payable at any branch of the State Bank of India throughout India, or by Demand Draft or at par multi city cheques payable to DENTAL BUSINESS SOLUTIONS at Ahmedabad.

The Foreign remittance can be made through SWIFT CODE SB IN IN BB 417 A/C NO 30450928544. IFSC CODE : SBIN0001043 Account Number 30450928544. The application duly filled must be accompanied with details of the payment made.

Application shall be accompanied with the proof of payment of the requisite fee. The applicant will have to undergo training comprising 12 hrs of lectures, demonstration and hands on and a written test at Ahmedabad . The training will be conducted over the week ends on Saturday and Sunday. The candidates can choose their dates for the training, subject to availability. A maximum of 8 candidates will be taken at a time for the training to ensure personal attention. On completion of the training, the candidates will be awarded. The training program is not compulsory. The applicant may decline the offer for training.
